The Institute of Mental Health is seeking a highly skilled and compassionate Senior Case Manager to join our team. In this role, you will be responsible for managing and coordinating the care of individuals with mental health conditions, ensuring they receive the highest quality of treatment and support. This position is crucial in providing comprehensive mental health services to our community.

📋 Tanggung Jawab Pekerjaan

Manage and coordinate the care of individuals with mental health conditions, ensuring they receive the highest quality of treatment and support.,Develop and implement individual treatment plans in collaboration with the multidisciplinary team.,Monitor and evaluate the progress of clients, providing regular updates to the care team and stakeholders.,Facilitate group therapy sessions and support groups to promote peer-to-peer support and community building.,Collaborate with healthcare providers, community organizations, and government agencies to ensure seamless care coordination.,Provide advocacy and support to clients and their families, ensuring their voices are heard and their needs are met.,Maintain accurate and confidential client records, adhering to all relevant laws and regulations.,Participate in staff meetings and training sessions to stay updated on best practices and new developments in mental health care.

🎓 Kualifikasi & Syarat

Bachelor's degree in Social Work, Psychology, or a related field.,Minimum of 5 years of experience in mental health case management, preferably in an EPIP (Early Psychosis Intervention Program) setting.,Strong knowledge of mental health treatment and intervention strategies.,Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families.,Experience in working with diverse populations and cultural sensitivity.,Proficiency in English, both written and spoken.,Knowledge of relevant laws and regulations governing mental health care in Indonesia.,Valid certification or license as a mental health professional in Indonesia.
